  • Can we choose region, environment boundaries, and network architecture?

  • Who inside the vendor can access patient data, and under what process?
  • Are access events auditable, reviewable, and limited by role?
  • Do support workflows expose PHI unnecessarily?

About data rights

  • Is our data ever used to train shared or general models?
  • What are the retention and deletion policies for inputs, outputs, and logs?
  • Can we export records and preserve our own governance trail?

About resilience

  • What happens during an incident, a vendor change, or a model swap?
  • Can we restrict workflows by user, workspace, or data source?
  • Do we have clear fallback paths if an AI system should not see a class of data?

Why patients should care too

This is not only an enterprise IT issue. Patients increasingly interact with digital intake tools, AI-assisted support flows, documentation systems, and automated coordination layers. They may never see the architecture behind those tools, but they still live with the consequences of how data is handled.

When organizations know who controls data, patients are better protected from unnecessary exposure, unclear reuse, and fragmented accountability. When they do not, trust erodes even if the tool appears impressive on the surface.
